Altogether Better Living Well, a collaborative of four organizations based in Sheffield, Hull, Wakefield and Feel Good Factor in Leeds has been successful in obtaining funding from the Silver Dreams Fund, a major initiative of the Big Lottery in association with the Daily Mail.
Altogether Better Living Well which aims to co-produce a new network of Older Living Well Champions to voluntarily support older people and their carers who live with long term conditions.
We plan to recruit and train older Living Well Champions who are keen to use their local knowledge, life experience, skills and understanding to reduce isolation and improve mental health and well-being for older people who are living with long term conditions. Living Well Champions will offer support, increase social contact and community engagement, create activities with and for older people, build self confidence and increase the voice of older people within health care pathways.
We intend to work closely with GP’s, Foundation Trusts, Local Authorities and Community organisations to share expertise and to integrate older people into the planning and delivery of services to improve quality and to enable older people to live life to the full.
The Leeds part of the project will be based in the Chapeltown area, we are currently recruiting volunteers to train up as Living Well Champions. Volunteers will receive training and support locally and have the opportunity to share their experiences with other Champions in the three pilot sites.
